We promote independent living skills for care leavers - health, education, training, employment, citizenship, budgeting, home safety, and relationships.
Emergency Referrals
We provide accommodation to support young people in need of an emergency referral, while their circumstances are assessed and if needed a more appropriate longer-term solution is found.

Accompanied Care Services
Specialist Supported Accommodation for Care Leavers & Unaccompanied Asylum Seeking Young People aged 16 - 21
Short-Term - Placement- up to three months Emergency / Unplanned: For young people experiencing family breakdown, foster care breakdown or remand to Local Authority care by courts.
Medium Term Placement – three months to one year
Planned Placement which extends beyond three months:
- The engagement and focus of our placements are to provide the young person with a supportive learning environment whereby they are able to meet the outcomes identified as part of their care and pathway plan.
- Long Term Placements focus on transitional planning that leads beyond the young person’s 17th birthday and prepares them to develop skills ready for independent living
We offer services for young people who require intensive supervision and interventions to manage their presenting risks in a safe, stable and monitored environment through the use of our behaviour modification programme.
CARE SERVICE USERS
Accompanied Care specialises in providing superior care and supported accommodation to care leavers, looked after & asylum seeking young people aged 16 - 21. We provide a young person centric approach, which ensures their transition in to early adult life.
SUPPORTED ACCOMMODATION
All of our supported accommodation is staffed up to 24 hours a day, we support young people between the ages of 16 - 21 who may be vulnerable with varying degrees of support needs. We offer modern, high standard properties that are fit for purpose and young person friendly. Our properties are inclusive of wi-fi, games console, library and a house computer.
CARE PACKAGES
We offer a range of Holistic Care Packages that can help identify and meet the needs of young people, our service promotes the learning of new skills and interests; with the right support and guidance that will encourage and enable our young people to live independently.
We have established employment links within the local community and are able to offer our residents the opportunity of a paid work experience placement that could lead to a permanent job on completion.
We have links with local gas/electric engineers, a car garage and a local desert shop who would all be willing to offer our young people a work experience placement.

We focus on helping our young people to develop and maintain as much independence and control over their lives as possible, whilst developing and improving their self-confidence, self-esteem, motivation and well being. Our services are delivered by trained and experienced support workers and qualified professionals. Services can be provided 24hrs a day 365 days a year, with a staff member on site 24/7
Education & Employment Activities

Support to access apprenticeships, employment, education or training.

Careers interviews and career pathway planning sessions with a Level 6 Qualified Specialist Careers Advisor

Support completing application forms, guidance to complete CV's, interview skills, telephone skills, support with college courses or finding appropriate training.

Accommodation and Key work Sessions:

We will provide a clean, friendly and homely environment and accommodation – with a starter pack to provide you with your daily living essentials.

The accommodation will have staff on side 24/7 365 days a year.

All key workers can offer advice and support for any issues/problems/queries you may encounter during your stay at Accompanied Care.

Your key work sessions are your time to seek advice, discuss, complain or simply talk through your feelings

Accompanied Care offers accommodation within a clean, friendly and a homely environment.
Health

Registering with the local GP, dentist and optician etc

We will ensure you are aware of the location of the nearest hospital, NHS walk-in health centre, sexual health clinic, substance misuse service.

Accessing specialist counselling services, professional help and/or other agencies

Arranging and attending appointments.

Liaising with the social worker, doctors, family etc.

Dealing with emotional difficulties (stress/anxiety) (informal counselling)

Monitoring and supervising the use of prescribed medication though prompting

Arranging prescriptions and medication collection from the pharmacy.
Managing Budgeting and Independence

Assistance with budgeting for food, bills, clothing, recreation etc.

Assistance with welfare benefits, forms, bills.

All young people will be encouraged/supported to open a bank account if they do not already have one.

Assistance with preparing shopping lists, shopping and preparing meals

Understanding the importance of nutrition and hygiene

Help with cleaning a home/room, making a bed, laundry, ironing etc.

Prompting and guidance with daily personal hygiene routines

How to keep safe in the kitchen, support if needed to operate & maintain kitchen equipment such as washing machine, dishwasher, oven and the hob.

Advise on fire safety, food hygiene and storage

Support developing life skills and confidence.
Partnerships/ Working Together:

We will continue to build partnerships with professionals/agencies to give you access to a network of support.

We will work closely with the local authority and social work team to ensure you have access to the services that meet your needs

Assisting with religious and cultural observances

Assisting with organising social events and outings

Every month your key workers will submit a report about how you have settled in, your progress, opportunities identified, your strengths and weaknesses and will provide you with an opportunity to give your views and comments.
