Accompanied Care Referrals

Accompanied Care accommodates young people that are making the transition from residential or foster care to independence. We consider all young people from different backgrounds. A needs and risk assessment is completed for every young person referred to the service. We will consider emergency placements but this would follow the same admissions procedure.

We will not consider referrals for young people where we could not meet their needs effectively. Matching of young people will be an important consideration when looking at referrals.

Young people must be 16+. Referrals must be completed by Social Workers or other Local Authority staff who are authorised to do so and are then forwarded to ourselves at Accompanied Care. In the first instance, an expression of placement interest can be made either by telephone, email or via our website. This will be followed by a discussion which will take place between the referrer and a member of the management team who will assess the young person's needs and suitability. Placements will not commence without a full risk assessment being undertaken.
